Incoming 6th grade immunization requirements

Dear Parents and Guardians,

As we prepare for the upcoming school year, we want to remind you of the New Jersey Department of Health’s immunization requirements for all students entering 6th grade. These immunizations are essential for your child’s health and are mandatory for school attendance.

Effective in the state of New Jersey, all students entering 6th grade who are 11 years old or older must receive the following vaccines:

  • Tdap (Tetanus, Diphtheria, and Acellular Pertussis) booster

  • Meningococcal conjugate vaccine (MCV4)

Your child must receive these immunizations once they turn 11 years old and prior to the first day of 6th grade. If your child turns 11 during the school year, they are expected to receive the vaccines within two weeks of their birthday.

Please submit official documentation of these immunizations to the school nurse jodie.witt@shrsd.org by August 20, 2025. Students who do not meet these requirements may be excluded from school until documentation is received.

If your child has a medical exemption, a physician's statement must be submitted in compliance with New Jersey law. Please note that religious exemptions must be documented in writing and comply with state policy.

We encourage you to schedule an appointment with your child’s healthcare provider if these vaccines have not yet been administered. If you have already completed these vaccinations, please send the updated immunization record to the school nurse via email.
